Employed to implement the HSE requirements of the company on the project to a standard that will be acceptable to Leighton International Australia and reporting to the Project Manager. Keeping all records as per ISO standards and requirement. To promote HSE at all levels at site and to the surrounding villages. Also implementing a system that will in-cooperate the Indian government HSE and the company HSE requirement.

I was also in-charge of security. I created a security plan and implemented it through a Security Manager reporting to me.

Transferred from QAQC to the Safety Department on the 9th August 2002 to assist the Safety Manager as a Safety and Security Officer as one of the safety Officers to implement the Projects Safety Procedures and Plan and the Occupational Safety and Health 1994 procedures and also to assist in the security matters of the company at site. SAFETY ASPECT:

QC Supervisor

Employed to assist the QC Engineer at the Kuala Lumpur Sentral Project in the capacity of a QC Inspector to carry out site inspection on various C&S works of the project which includes carrying out sport checks on site for:

- Rebar – assembly and the condition of the rebar
- The formwork – sufficient cover, verticality and condition of the wood or metal
- Concrete – type of concrete being used, slump is within tolerance, test cube are made accordingly and concrete is being carried out correctly.

In addition to these duties, I was in charge of auditing the QC documents on the works for the C&S Production Department, the M&E Production Department and the Railway Department.

Employed by an infra-structure company, I was in charge of laborers, sub-contractors and the machineries. Delegating specific works to the masons and carpenters ( the construction of manholes and making of formworks for varies concreting works) inspecting the sub-contractors work, making sure that the work being carried out is in accordance with the specifications. Carrying out earth works with machineries such as excavating for sewage VC pipes, drainage, sewage manholes, drainages sumps, cutting of slopes, removing excess earth from work site with lorries, excavating for retaining wall, ground excavation to sub-grade level for road works. Laying of crusher run for road works and doing all other concrete works.

Was also in charge for liaison with the Resident Engineer and Main Contractor's representative regarding drawing changers, design changes, suggestion made to alter design to reduce cost as part of site problem solving as some of the other duties entrusted to myself.

QC Inspector and Site Supervisor

Reporting to the Construction Manager to perform a dual role as a QC Inspector and a Site Supervisor on the 2.4 Billion Kuala Lumpur Monorail Project (Phase 1).

QC INSPECTOR

As an QC Inspector, I was responsible for verifying specifications and drawing compliance of pre-cast concrete works i.e. monorail guideway support beams and geotechnical construction works i.e. deep bore and micro-pile foundation including pile caps. This incorporation carrying out and signing off in-process and final inspection as per detail checklist.

Principle Inspection item
- Steel mould dimensional checks.
- Per-stressed and fibre – optic duct positioning.
- Rebar- size, dimensional and quality per beam check.
- Crosshead beam to support column connection points.
- Precast items identification and reference tagging verification for storage.
- Survey, set out and as-built coordination activities.
- Incoming material quality and quantity verification.

SUPERVISORY ASPECT

Site Supervisor incorporated schedule coordination and monitoring of daily works of sub-contractors i.e. mainly daily progress records, manpower – material and plant requirements, quality and safety policies implementation, review of bore-log records, concrete quantity estimation and cube test analysis, resolution of daily problems and planning including liaison with client and consultant representative.

JOHN HOLLAND (M) SND BHD – LRT START PROJECT
AUGUST 5, 1996 – FEBRUARY 6, 1997
Quality and safety Officer

Employed as an Quality and Safety Officer reporting to the Quality and Safety Coordination associated with the track works for the Light Rail Transit Project.

Conducting incoming inspection of procurement items principally associated with special process:
- Termite welding application for track joining.
- Installation of geo-textile material.

Ensure that installation process complied with method statements, specification and manufacturers criteria. Incorporated the application checklist requirements including indexing and filing as project rack sequence for eventual QA auditing.

The above were pre-final completion aspects and regarded as critical processes which required certification. Associated control activities are:

- Compilation of as-built survey records.
- Welding preparation and calibration post-grinding.
- Ballast stone installation and sieve test monitoring.
- Ensuring the handling and storage characteristics.

In view of the elevated nature of works, I was also coordinating safety tool box talks, ordering of safety materials, conducted safety audits at site, attended safety committee meeting for the project, delegation of works and site safety inspections.

Site Clerk / Site Administrator / Site Supervisor

Initially employed as a Site Clerk / Administrator assisting and reporting directly to the Project Manager accountable for the civil i.e. piling and structural foundation aspects, for the RM 4.7 Billion ( Phase 1 and 2) Light Rail Transit (STAR) project.

Responsible for collating operational project records i.e. time keeping, plant and material resources, construction activity report, purchasing and office supplies, etc.

Was given internal training in utilizing the company Document Management system which further to the above included the implementing and maintaining of applicable drawing control, register updates, records of drawing changers and distribution.

Once the above was established, I was later assigned to the Project Engineer to fulfill the Site Supervisory aspects:

- Plant Mobilization and spoil excavation disposal.
- Welding of bore-pile rebar cages and splicing.
- Concrete quantity and trimmie pipe length calculation.
- Monitoring of bore-logs and concrete cube test results.
- Field liaison with sub-contractors and consulting engineers
